A diagnostic trouble code is the ECU recording that a monitored test failed under specific enabling conditions. The code describes the circuit or system that failed the test — not necessarily the component responsible. That is the single most important idea in code-based diagnosis.
Codes exist in states: pending (failed once), confirmed (failed on two consecutive trips), and permanent (emissions-related, cleared only by the ECU after successful tests).
